Services

The Center for Community Health (CCH) offers a new vision of outpatient care for children and adults by providing multispecialty preventive, diagnostic, and therapeutic services all in one location. Our six-story, 400,000 square-foot facility is located on the corner of 8th Avenue and 6th Street in Park Slope, Brooklyn.

Ambulatory surgery

With a record 12 operating rooms dedicated to same-day outpatient surgeries, the CCH is fully equipped to perform a variety of outpatient surgical procedures. Our multispecialty clinical teams — comprised of NewYork-Presbyterian Brooklyn Methodist and world-class Weill Cornell Medicine physicians — use minimally invasive techniques to make the smallest incision possible. This approach allows patients to heal quickly and recover in the comfort of their own home. Additionally, patients find that same-day outpatient surgery allows for reduced costs and travel time, as well as ease in scheduling compared to inpatient surgery. Our surgical team carefully reviews each case and discusses expectations with families to ensure that patients are selected appropriately for outpatient surgery and don’t require extended recovery or inpatient surgery.

Each state-of-the-art operating room includes minimally invasive capability (eg, laparoscopic and laser), as well as an integration system where surgical staff can access device controls, data, and video from one central command station — helping to increase efficiency and safety during procedures. The CCH assigns all patients who are scheduled for outpatient surgery a “nurse-of-the-day,” who will care for them both before and after their surgery.

Cancer Center

Recognized by the American College of Surgeons as a Commission on Cancer (CoC) accredited cancer program, patients at the CCH’s Cancer Center have access to a full range of outpatient cancer services including the latest anti-cancer drugs, targeted therapies, immunotherapies, minimally invasive outpatient surgical procedures, and support and educational resources. Every effort is made to make a patient’s visit as comfortable and efficient as possible, whether visiting our chemotherapy infusion center or participating on one of our cutting-edge clinical trials.

Our CCH cancer care team includes NewYork-Presbyterian Brooklyn Methodist and Weill Cornell Medicine cancer surgeons, medical oncologists, radiologists, pathologists, oncology nurses and nurse practitioners, social workers, and nutritionists. We offer prevention, screening, diagnosis, and outpatient treatment services for a variety of cancers including:

Infusion center

At the CCH’s dedicated on-site infusion center, we are able to offer outpatient chemotherapy and other infusion therapies six days per week. Our infusion center has an adjacent laboratory, allowing our trained pharmacists and pharmacy technicians to create pharmacy preparations on-site and deliver medications efficiently with minimal patient wait time. Additionally, we are one of the few Brooklyn locations to offer cold cap therapy, which minimizes hair loss during treatment.

Cancer clinical trials

Through our partnership with NewYork-Presbyterian and Weill Cornell Medicine, patients at the CCH’s Cancer Center have the opportunity to participate in one of our cutting-edge cancer clinical trials. NewYork-Presbyterian Brooklyn Methodist Hospital is a site for the NRG Oncology Group, which brings together the National Surgical Adjuvant Breast and Bowel Project, the Radiation Therapy Oncology Group, and the Gynecologic Oncology Group — each recognized as an international leader in cancer research. Through our relationship with Weill Cornell Medicine, we are also a member of the prestigious Alliance for Clinical Trials in Oncology, where patients may have the opportunity to participate in a clinical trial of a promising new gynecologic cancer therapies.

Patient navigation

Patients understandably have many questions throughout their cancer journey. Our CCH navigation team serves as the patient’s guide and advocate throughout the treatment process, connecting them to the resources they need — whether it be a second opinion, questions about a clinical trial, or participation in a cancer support group.

Cardiology

The CCH is committed to a healthier Brooklyn and recognizes that heart disease and conditions that negatively affect the heart—like hypertension, obesity, and diabetes—are among the most common health conditions facing Brooklynites. Our outpatient cardiology practice is increasing access to care in the region by offering the full range of outpatient cardiac services from screening and diagnostic procedures, to managing chronic heart conditions and performing minimally invasive surgery. With a specialty suite dedicated to cardiology, the CCH places special emphasis on preventing heart disease, promoting heart health, and reducing hospitalizations.

Our award-winning Weill Cornell Medicine physicians offer care in all areas of cardiology including:

Preventive cardiology. Our preventive heart specialists work with you to create a targeted treatment plan to reduce your risk for heart attack, stroke, bypass surgery, or angioplasty.

Cardiovascular testing. We offer advanced screening and diagnostic tests such as coronary CT and MRI scans, 3D transesophageal echocardiography, nuclear stress testing, and arrhythmia screening.

Structural and valvular heart disease. We offer minimally invasive catheter-based procedures for patients with structural heart defects.

Interventional cardiology. Our interventional cardiologists perform procedures such as cardiac catheterization, stents, embolic protection, valve repair, and atherectomy.

Electrophysiology. Our cardiac electrophysiologists perform electrophysiology studies, cardiac ablation, and implantation of cardioverter-defibrillators, loop recorders, and pacemakers.

Hypertension. We offer comprehensive evaluations to discover the causes of hypertension and help manage your cardiovascular risk factors including ambulatory blood pressure monitoring, hormonal profiling, pulse wave analysis and complete cardiovascular evaluation.

Heart failure. The CCH has a dedicated program for patients with congestive heart failure, which provides monitoring, treatment, and follow-up care — all while working in conjunction with NewYork-Presbyterian’s ventricular assist device and heart transplant programs.

Cardiac surgery. Our cardiac surgeons use minimally invasive techniques to treat a variety of heart conditions in one of our 12 state-of-the-art operating rooms.

Digestive diseases

Our multidisciplinary digestive diseases care focuses on identifying and treating disorders of the gastrointestinal tract, pancreas, and liver. With a specialty suite dedicated to gastroenterology, the CCH offers screening and diagnostic procedures like colonoscopy and endoscopy, management of chronic gastrointestinal conditions, and minimally invasive outpatient surgical procedures for benign (non-cancerous) digestive diagnoses.

We assembled a unique group of award-winning Weill Cornell Medicine specialists including gastroenterologists (doctors who specialize in digestive disorders), hepatologists (doctors who specialize in liver diseases), minimally-invasive surgeons, gastrointestinal radiologists, nutritionists, and psychologists to deliver multidisciplinary care. The CCH outpatient digestive diseases practice offers care in all areas of gastroenterology including:

Bariatric surgery. We perform minimally invasive weight loss surgery in one of our 12 state-of-the-art operating rooms.

Colon cancer screening. We offer screening procedures including colonoscopy, flexible sigmoidoscopy, and high sensitivity stool tests, as well as genetic counseling/testing to assess for colon cancer risk.

Heartburn and GERD. We diagnose and treat disorders of the esophagus and stomach, including gastroesophageal reflux disorder (GERD).

Hemorrhoids. We diagnose and treat both internal and external hemorrhoids and work with you to make dietary changes.

Inflammatory bowel disease (IBD). We diagnose and treat both Crohn’s disease and ulcerative colitis, and offer intravenous biologic therapies at the CCH’s on-site infusion center.

Irritable bowel syndrome (IBS). Our gastroenterologists and nutritionists work with you to create a targeted treatment plan.

Liver diseases. Our hepatologists diagnose, treat, and manage a variety of chronic liver diseases including fatty liver disease, cirrhosis, and hepatitis. We work in conjunction with NewYork-Presbyterian’s liver transplant programs as needed.

Motility disorders. We diagnose and treat swallowing disorders (achalasia) and diseases of the esophagus.

Pancreatitis. We perform a very specialized procedure called endoscopic retrograde cholangiopancreatography (ERCP) to remove stones that may be blocking the pancreatic and bile ducts.

Peptic ulcers. We use multiple modalities to treat peptic ulcers that are found on the inside of the stomach (gastric ulcers) or the inside upper portion of the small intestine (duodenal ulcers).

Orthopedic medicine

The CCH offers the full spectrum of orthopedic and sports medicine services for adults and children, from the conservative management of orthopedic conditions to advanced minimally invasive outpatient surgeries. Our unique team includes specialists in adult and pediatric orthopedic surgery, emergency medicine, sports medicine, pain management, and spine surgery. With a dedicated orthopedic suite, the CCH is able to provide coordinated, comprehensive, multidisciplinary outpatient orthopedic care including:

Hand, arm, & shoulder treatment. We offer a variety of treatments for conditions of the wrist, elbow and shoulder — ranging from conservative treatments to outpatient arthroscopic surgeries.

Hip preservation. We offer non-surgical rehabilitation services including the use of platelet-rich plasma, as well as minimally invasive outpatient hip arthroscopy.

Joint replacement. We offer same-day joint replacement surgery for selected patients.

Knee surgery & cartilage restoration. We perform minimally invasive outpatient procedures like arthroscopic surgery for meniscus tears, ACL ligament tears, cartilage wear, and patella problems.

Orthopedic rehabilitation therapy. We offer physical and occupational therapy to help patients recover from an orthopedic injury or illness.

Pain management. We evaluate and treat patients with both acute and chronic pain, focusing on decreasing pain and the use of potent drugs, as well as increasing patients’ well-being.

Pediatric orthopedics. Our fellowship-trained pediatric orthopedic surgeons provide comprehensive and compassionate outpatient care for children with a wide range of orthopedic conditions.

Spine surgery. We specialize in the diagnosis and treatment of spinal disorders.

Sports medicine. We offer comprehensive evaluation, diagnosis, and treatment services for all types of sports-related injuries — from elite athletes, to weekend warriors and childhood sports injuries.

Diagnostic Imaging Center

The CCH is home to one of the most advanced imaging centers in New York City. Radiologists at our Diagnostic Imaging Center produce the highest quality images and immediately incorporate the results into a patient’s electronic medical record, helping to accurately and efficiently diagnose a variety of conditions. Our patient-friendly setting includes a spacious waiting room and private changing rooms with personal televisions.

The Diagnostic Imaging Center’s sophisticated equipment allows for images of bones and soft tissue, as well as the brain and the circulatory system. During diagnostic or therapeutic interventions, doctors use advanced imaging registration software to guide procedures by overlaying previous imaging scans on real-time imaging. Our advanced technology includes: The Center will include:

  • A state-of-the-art 3 Tesla MRI system. This advanced system boasts greater detail, no harmful radiation, and an open wide tunnel allowing for greater comfort than previous technologies.
  • Breast imaging. We offer a variety of tools for breast screening including digital and 3D mammography, breast ultrasound, and breast magnetic resonance imaging (MRI).
  • A state-of-the-art digital PET-CT scan.
  • X-ray.
  • Fluoroscopy.
  • Ultrasound.

Pre-Anesthesia Evaluation Center

Patients who are scheduled for minimally invasive outpatient surgery will undergo an evaluation by one of the anesthesiologists at the CCH’s Pre-Anesthesia Evaluation Center. The pre-anesthesia evaluation will take place within 48 hours prior to the day of surgery and entails a physical exam and a comprehensive medical history including a list of medications and allergies. This gathering of information helps our doctors to identify any risk factors before your outpatient surgery and perform additional testing as necessary. The plan that is developed by the Pre-Anesthesia Evaluation Center at the CCH is shared with your surgeon and anesthesiologist and helps to control your pain and deliver anesthesia as safely as possible.

The Breast Center (coming in 2023)

Set to open its doors in 2022, the CCH’s Breast Center will provide patients with comprehensive breast care including routine mammography and diagnostic studies, minimally invasive outpatient biopsy techniques, state-of-the-art surgical services, breast reconstruction, and consultative second opinion services. With advanced imaging technology and our expert team of breast specialists, we take a proactive approach to prevent, screen, and diagnose benign and malignant diseases of the breast.

Our multidisciplinary treatment approach often includes a combination of treatment modalities, which results in more targeted therapy, less harm to surrounding tissue, and reduced treatment time. Our multidisciplinary breast care includes:

Breast cancer diagnosis & screening. We use the most accurate imaging tools to detect breast cancer including digital and 3d mammography, breast ultrasound, and breast magnetic resonance imaging (MRI). and prevention, diagnosis, and treatment

Breast surgery. We offer treatment for breast lumps, abnormal mammograms, nipple discharge, and breast cancer including sentinel node biopsy, breast-conserving surgery (lumpectomy), and mastectomy.

Medical treatment. Medical oncologists at the CCH use precision medicine to create targeted treatments based on your tumor’s specific molecular features. In our infusion center, we offer outpatient chemotherapy and cold cap therapy, which minimizes hair loss during treatment.

Radiation therapy. Our radiation oncology team uses a variety of modalities to target your breast cancer tumor including precision external-beam radiation therapy, prone radiation therapy, and internal radiation therapy.

Women's health and reproductive medicine center (coming in 2023)

The CCH offers prevention, diagnosis, and treatment services for a variety of conditions that are common among women of all ages. With experts in obstetrics and gynecology, primary care, internal medicine, and surgery, we provide a broad range of services to women including:

Breast cancer. We offer comprehensive breast care including routine mammography and diagnostic studies, minimally invasive outpatient biopsy techniques, state-of-the-art surgical services, breast reconstruction, and consultative second opinion services.

Gynecologic care. We offer routine annual gynecologic care including preventive care, cancer screening, birth control consultations, treatment for premenstrual syndrome, and menopause management.

Maternal and child health. We offer a sophisticated range of prenatal testing options for expectant mothers and take a family-centered care approach to childbirth, allowing the family to experience the entire birth process — labor, delivery, and recovery.
