Community Hearing Health Collaborative (CHHC)

Yes, you can receive audiology services at other locations and still enroll in NewYork-Presbyterian Hospital’s Community Hearing Health Collaborative Program.

For referrals, ask your doctor to complete the CHHC referral form or to contact us via [email protected] or 212-305-3340.

Our bilingual community health workers maintain a relationship with the families to:

• Engage them with medical educational and social services
• Visit families at their homes to provide one-on-one support
• Advocate and accompany patients to appointments as needed

Yes. There is no cost to services provided by Community Hearing Health Collaborative.

The program is for children 0-3 years old who fail infant hearing screening or have received a hearing loss diagnosis.
