How can I refer my child to the Community Hearing Health Collaborative program?
Can the community health workers accompany patients to appointments?
Our bilingual community health workers maintain a relationship with the families to:
- Engage them with medical educational and social services
- Visit families at their homes to provide one-on-one support
- Advocate and accompany patients to appointments as needed
Is this service free?
Yes. There is no cost to services provided by Community Hearing Health Collaborative.
How old does my child have to be to qualify for the program?
The program is for children 0-3 years old who fail infant hearing screening or have received a hearing loss diagnosis.
