Announcements/Deadlines
- The PMO will be scheduling Root Cause Analysis with each onsite Homecare and will host RCA if needed form all sites.
- Please complete the templates for RCA template with action plans and submit to the PMO.
- All onsite Home Care sites are collaborating with NYP Queens to form a Rapid Response Team to facilitate discharge planning and confirmation of home care services.
