Cancer Screening and Prevention, Breast Care, and Care Coordination
We know it can be overwhelming to keep track of everything you need to do as part of your health maintenance and cancer care. To guide you , our patient navigators at NewYork-Presbyterian Lawrence Hospital will coordinate your care, ensuring that you have timely, appropriate access to the right care team and services. They will also make sure that your primary care provider is aware of your treatment plan.
Our Cancer Screening Navigator, working closely with your primary care provider, will provide education about your cancer screening options and arrange the appropriate exams you need, after a thorough discussion with you.
Our Breast Care Navigator will work with our community partners and identify women who are not up-to-date on their breast cancer screening. The navigator will offer assistance to remove any barriers to care and guide you to the screening examinations you need — such as mammography and, for some women, breast ultrasound.
If cancer is found, our Cancer Care Coordination Navigator will assist you in obtaining the medical information, pathology slides, and radiology results needed for your consultations and care; facilitate the scheduling process for appointments, tests, and follow-up care; act as a liaison between you and your medical team during your treatment; and refer you to community resources.
Every step of the way, the patient navigators at NewYork-Presbyterian Lawrence Hospital are here to provide you with guidance and support.
