Referral Process
What is the NYP Health Home Referral Process?
- Complete the electronic Health Home referral form
- If it is an urgent / same day referral, please call 212-342-0454 and someone will assist you.
- Someone from the Health Home team will contact you:
- If the patient is eligible for the NYP Health Home, the Health Home team will assign the patient to one of our collaborators.
- If the patient is not eligible for the NYP Health Home and / or the patient is already engaged with another Health Home, the NYP Health Home Team will reach out to another lead Health Home to connect you.
- For urgent referrals, the Health Home team will contact you within the same business day.
- For non-urgent referrals, the Health Home team will contact you within 2 – 5 business days.
- Follow up with the Health Home partner within 2 business days to review case to ensure connection was made.
All referral updates and communication will be via email and/or phone.
