Going Home

When You Are Discharged

Your doctor will determine when you are ready to leave NewYork-Presbyterian Westchester (formerly Lawrence Hospital) and will write the necessary orders. It is often advisable to ask a family member or friend to assist you on the day of discharge. On your day of discharge, it is expected that you arrange to leave by 11:00 a.m. for the benefit of a patient waiting to be admitted.

Your physician and nurse will give you discharge instructions and answer any questions you have about managing your treatment and medications once you are home. If you are confused or unsure about what you need to do, what medications you must take, or if you have to restrict your diet or activities, don’t be afraid to ask and take notes. Be sure you understand any instructions you have been given before you leave the hospital.

Going Home

When your doctor feels that you are ready to leave the hospital he or she will authorize a hospital discharge. Please speak with your nurse about our discharge procedures.

Here are few tips to make the discharge process run smoothly:

  • Be sure you and/or your caregiver have spoken with a discharge planner and that you understand what services you may need after leaving the hospital.
  • Verify your discharge date and time with your nurse or doctor.
  • Have someone available to pick you up.
  • Check your room, bathroom and bedside table carefully for any personal items.
  • Make sure you or your caregiver has all necessary paperwork for billing, referrals, prescriptions, etc.

Escort Service

When it is time for you to leave the hospital, you may be escorted to the Cashier’s Office on the first floor to take care of remaining financial paperwork. At this time, please present your receipt for any valuables stored in the hospital safe and request that they be returned to you.

After Discharge An Expression of Gratitude/Thanks

NewYork-Presbyterian Westchester (formerly Lawrence Hospital) does not permit gratuities. It is our mission to exceed your expectations. If the staff has gone out of their way to help you in some way, a letter of thanks is greatly appreciated. You may also recognize any staff member through our “Big Heart” employee excellence program. “Big Heart” recognition cards are available at the nurse’s station on every unit. Many patients want to express appreciation for the care they received in a more formal manner. Soon after your discharge, on behalf of NewYork-Presbyterian Westchester (formerly Lawrence Hospital), an independent company, Press Ganey, may mail you a confidential patient satisfaction survey. Please take the time to share your opinions about your hospital stay. Your feedback is an important part of our goal of improving the care and services we provide.

Home Care Options

If your physician prescribes Certified Home Health Care services for you, your Case Manager will assist you in accessing such services.

Lawrence Home Care of Westchester (LHCW) is the hospital’s Certified Home Health Care service and works closely with your physician to ensure continuity of care. If you wish, and your insurance carrier agrees, your Case Manager will arrange for your services to be provided through LHCW. However, you do have the right to choose the Certified Home Health Care service of your preference and if you do require home assistance your Case Manager will provide a list of agencies for you to choose from.
