Community Health Needs Assessment & Service Plan

NewYork-Presbyterian is committed to understanding and improving the health and well-being of the communities we serve. As part of this commitment, we conduct comprehensive Community Health Needs Assessments (CHNA) every three years to identify community health priorities, concerns, and strengths across the New York City metropolitan area.

The CHNA process combines rigorous data analysis with direct input from community members, organizations, and local partners in our service areas to provide a clear and current picture of the factors shaping health outcomes. Insights from this process then guide our Community Service Plans (CSP), which outlines how NewYork-Presbyterian directs its community programs, partnerships, and resources to address these identified health priorities. The CSPs align with the New York State Prevention Agenda and describe how we work with community partners to advance our shared goals and track progress over time.

Our community health efforts span New York City and Westchester County and are shaped by ongoing partnerships with residents and local collaborators. We make our Community Health Needs Assessments and Community Service Plans available to promote transparency, accountability, and a shared understanding of how we are working together to improve community health.
