Who can be enrolled in the NewYork-Presbyterian Health Home?
We accept any patient receiving care from the NewYork-Presbyterian/Columbia University Irving Medical Center - Comprehensive Health Program (CHP) or the NewYork-Presbyterian/Weill Cornell Medical Center - Center for Special Studies (CSS) who are enrolled with one of the following Medicaid health plans:
- Affinity by Molina Health Plan
- Amida Care
- Dual eligible Medicaid/Medicare
- Emblem Health-HIP (Health Ins. Plan of GTR NY)
- Empire BCBS Health plus Medicaid
- Fidelis (NYS Catholic Health Plan)
- Healthfirst NY
- Medicaid FFS
- United Healthcare Community Plan
- VNSNY Choice
- Wellcare
