Home monitoring helps you manage your health from home. You may need to make lifestyle changes to stay healthy and follow your doctor’s instructions about diet, physical activity, and medication. It’s also helpful for your healthcare team to track certain vital signs and symptoms on a regular basis.
With NewYork-Presbyterian Hospital’s Remote Patient Monitoring Program, your care team can remotely monitor your vital signs and other health measurements.
Personalized Care
You will be asked to take specific measurements and take surveys about how you feel.
The hospital has arranged for a nurse to follow your responses, and if there is anything concerning, they will contact you and a NewYork-Presbyterian physician.
Comfort & Convenience
You will take certain vital signs from the comfort of your home. You can take comfort in knowing a care team of nurses will be monitoring the health information you send.
During Monday – Friday, 9 am – 5 pm EST, our nurses will actively monitor your logged measurements and survey responses when devices are in pairing range of the video tablet. Outside those hours, you have the option to contact them if you have any concerns, but they will not be actively monitoring your measurements.
Devices are wireless, so you can move about your house freely and comfortably.
Please remember to take vitals after you have taken your medications.
Easy-to-use
Based on your health status, some or all of the following devices will be delivered to your home, and the Current Health Pulse team will take you through the set-up process step-by-step.
- A video tablet
- Blood pressure cuff
- Weight scale
Method of installation
Remote installation (The kit is shipped to your home and installed over the phone.)
Steps to Enroll:
- Accept a call from the Current Health logistics team to schedule a date/time for installation within the next 48 hours. Someone will reach out to you at (574) 607-4870.
- Arrange for yourself or someone else to be at home at the scheduled time to have the devices delivered.
- Follow daily instructions on taking measurements and completing survey questions.
