NewYork-Presbyterian Launches Hospital at Home to Deliver Care in the Comfort of Home

NewYork-Presbyterian celebrated the launch of its Hospital at Home program at a recent ribbon cutting.

NewYork-Presbyterian today announced the launch of Hospital at Home, a care program for certain medically eligible patients to receive hospital care in the comfort of their home, delivered by NewYork-Presbyterian teams.

“With NewYork-Presbyterian Hospital at Home, our goal is to create a positive healing environment where we can care for our patients and help them feel better, in the comfort of their homes,” said Dr. Maya Vankineni, medical director of the Hospital at Home program at NewYork-Presbyterian/Columbia University Irving Medical Center.

“We are so pleased to be launching NewYork-Presbyterian Hospital at Home, offering a unique experience for patients by extending our high-quality care into the home setting,” said Dr. Alexander Fortenko, medical director of the Hospital at Home program at NewYork-Presbyterian/Weill Cornell Medical Center.

To be eligible for the program, patients must be recommended and medically cleared by a physician to participate, live in Manhattan within 30 minutes of the NewYork-Presbyterian/Columbia University Irving Medical Center or NewYork-Presbyterian/Weill Cornell Medical Center campus, and have a clean, accessible home in addition to meeting certain clinical requirements. NewYork-Presbyterian Hospital at Home is voluntary and covered by Medicare. At-home patient services include remote monitoring, imaging, pharmacy support and the convenience of in-person and telehealth visits with the care team.

When the patient leaves the hospital, NewYork-Presbyterian Hospital at Home will give the patient a device that monitors their vital signs. A NewYork-Presbyterian care team member will go to the patient’s home twice a day to check their vitals, give them their medication, and evaluate their progress. The patient will have a daily telehealth visit with their NewYork-Presbyterian Hospital at Home medical provider during their participation. The care team will then help patients and their loved ones understand and plan for the next steps after they are discharged from the program, including planning for upcoming doctors’ visits, changes to medication regimen, and what to look out for to make sure the patient stays healthy.
