Parkinsons Care Program Assistant

Patients, families and medical providers who work together can improve overall quality of life for patients living with Parkinson's disease. Under the supervision of the Parkinson's patient care coordinator, Alzheimer's/Dementia coordinator, the Parkinson's Care Volunteer will assist the coordinators in fulfilling a range of administrative and clinical needs to best serve the patient. On any given shift, you may conduct follow-up calls to patients seen by our neurologists, handle calls from the American Parkinson's Disease Association I&R hotline, provide crisis intervention services, link patients or families to support services, and prepare a resource guide. You may conduct mini evaluations for new patients to assess their level of cognitive functioning and quality of life. You will also have the opportunity to co-facilitate support groups, be responsible for composing monthly newsletters and medical letters and attend movement classes.

We would love to hear from you if you are: compassionate, able to take initiative and work independently in a multidisciplinary team, an excellent communicator, capable of managing multiple projects simultaneously in a fast paced environment, proficient in Word and Excel, have previous case management, group-work, clinical, administrative, bilingual and/or care coordination experience a plus

Minimum Commitment: Two four-hour shifts per week for a minimum of six months.

Times: Monday - Friday 9:00 a.m. to 1:00 p.m. or 1:00 p.m. to 5:00 p.m
