Specialized, long-term care

Patients with chronic kidney disease (CKD) and a history of transplant experience may face health challenges that require the care of expert nephrologists and transplant nephrologists. These conditions include organ rejection, certain infections unique to transplant patients, new-onset diabetes, hypertension, and an increased risk of heart disease. At NewYork-Presbyterian, we offer personalized and comprehensive long-term care for CKD patients with a history of kidney transplant. We recommend that you:

  • See your transplant team regularly for follow-ups
  • Follow the recommended schedule for lab tests and clinic visits to make sure that your kidney is working properly
  • Take your anti-rejection medications daily in the proper dose and at the right times, as directed by the transplant team, to keep your body from rejecting your new kidney
  • Follow a healthy lifestyle, including a proper diet and exercise

Kidney Care Programs & Centers 

Hypertension Centers 

NewYork-Presbyterian’s Hypertension Centers, located at NewYork-Presbyterian/Columbia University Irving Medical Center and NewYork-Presbyterian/Weill Cornell Medical Center, provide personalized clinical therapies for patients with hypertension. The Hypertension Centers have been designated by the American Heart Association (AHA) as a Comprehensive Hypertension Center, a distinction for institutions that meet or exceed standards for the research-based treatment of hypertension. 

Center for Glomerular Disease

Created in 2000 as the first of its kind in the United States, our Glomerular Center is dedicated to caring for people with rare diseases of the Glomerulus. The Center of Glomerular Kidney Diseases is a collaboration between members of the nephrology and renal pathology divisions at Columbia University.

Kidney Genetics Clinic

The Kidney Genetics Clinic at NewYork-Presbyterian/Columbia University Irving Medical Center utilizes a multidisciplinary team of clinicians and genetic counselors, as well as state-of-the-art diagnostic techniques, to provide comprehensive genetic services for adult patients with rare and inherited forms of kidney disease.

NewYork-Presbyterian/Columbia University Irving Medical Center and The Rogosin Institute are both recognized as Centers of Excellence by the Polycystic Kidney Disease Foundation.

Pediatric Nephrology Care

Pediatric kidney diseases can affect the quality of life and growth in children and cause serious complications if not treated properly. At NewYork-Presbyterian, we provide compassionate, expert care for newborns, children, and young adults with acute and chronic kidney disease. We focus on building ongoing relationships with children whose kidney conditions are chronic or progressive, as well as their families, and often care for them over many years. If your child needs a kidney transplant, we have extensive experience performing this complex procedure and with a track record of excellent outcomes. 

Kidney Transplantation Programs

For patients with chronic kidney disease and are approaching end-stage renal disease (ESRD), kidney transplant offers an alternative to dialysis. At NewYork-Presbyterian, we provide kidney transplant services at both NewYork-Presbyterian/Columbia University Irving Medical Center and NewYork-Presbyterian/Weill Cornell Medical Center.

  • The NewYork-Presbyterian Columbia Transplant Program is a recognized leader in kidney transplantation and performs over 200 transplants every year. Our goal is to maximize access to kidney transplantation so patients can live healthier lives. The kidney transplantation programs provide pre- and post-transplant care for patients and counseling for family members, including opportunities for living donation and donor swap programs. 
  • The NewYork-Presbyterian Weill Cornell Medicine Transplant Program, in cooperation with the Rogosin Institute, was the first kidney transplant program established in New York State more than five decades ago. Since that time, over 5,000 kidney transplants have been performed. 

The Rogosin Institute

The Rogosin Institute has long been recognized as one of the premier centers for the diagnosis and management of kidney disease in the country, recognized for its unique approach to the prevention, detection, and management of chronic and end-stage kidney disease. Rogosin cares for patients at locations throughout Manhattan, Queens, Brooklyn, and the Bronx and operates one of the largest home dialysis programs in New York City. 

Rogosin inpatient facilities at NewYork-Presbyterian/Weill Cornell include The Rogosin Institute Wing for treatment of kidney disease including kidney transplantation and The Rogosin Institute Dialysis and Apheresis Center. Rogosin Institute physicians care for patients in all clinical areas of NewYork-Presbyterian Hospital/Weill Cornell, including the Clinical Research Center (CRC).
