Francisco Maldonado came to New York City in his youth. Originally from Puerto Rico, Francisco and his family lived in the Bronx for a year before settling in Manhattan. "When I was young, me, my brother and three of my friends used to sing on the corner—108th Street and Broadway. People used to love it."
Now in his later years, Francisco still sings for others—especially the multidisciplinary staff of our Hospital at Home program. A patient in the program near the NewYork-Presbyterian/Weill Cornell Medical Center campus, Francisco is deeply grateful for the care he receives. "I'm very happy ... everyone treats me extremely nice," he shares. "They're good to me, and they make me comfortable."
Hospital at Home is a novel program that enables patients to receive NewYork-Presbyterian's expert hospital-level care from the comfort of their own home, supported by advanced technology, daily in-home visits, telehealth consultations, and 24/7 remote monitoring. This innovative model enhances recovery, improves the patient and provider experience, and helps reduce emergency department crowding. Patients receive twice-daily nursing visits, a daily video visit with their provider, and continuous access to a clinical care team. If a patient's condition changes, the team can respond quickly and coordinate a seamless return to the hospital when needed, ensuring uninterrupted, high-quality care across the continuum.
