What is Hospital at Home

Hospital at Home allows NewYork-Presbyterian patients to recovery and rest in the comfort of their own home, while still benefiting from the technology and expertise of a NewYork-Presbyterian hospital. This program helps to reduce emergency department crowding and is also associated with a better experience for both patients and providers. 

The program combines daily in-home visits from health care providers, virtual visits, and 24/7 remote patient monitoring. NewYork-Presbyterian Hospital at Home includes equipment to monitor patients, two visits by NewYork-Presbyterian nurses every day and a daily telehealth video visit with the Hospital at Home provider. Patients and caregivers have access to a clinical team 24 hours a day, 7 days a week. If a patient’s condition changes, the care team can respond quickly and, if needed, arrange a seamless return to the hospital.

Who is eligible

Eligibility for Hospital at Home is determined by a NewYork Presbyterian physician based on a patient’s medical needs and home environment. Hospital at Home is only available for certain medically eligible patients. Patients must live in Manhattan or The Bronx, and within 30 minutes of Columbia University Irving Medical Center or Weill Cornell Medical Center and have a home environment that supports safe delivery of hospital level care, including but not limited to reliable utilities and adequate space for medical equipment.

How it works

If a doctor clears a patient as medically eligible, when the patient leaves the hospital, NewYork-Presbyterian Hospital at Home will give the patient a device that monitors their vital signs. A NewYork-Presbyterian care team member will go to the patient’s home twice a day to check their vitals, give them their medication, and evaluate their progress. Caregivers and loved ones are included in the process and receive education and guidance throughout the patient’s recovery.

The patient will have a daily telehealth visit with their NewYork-Presbyterian Hospital at Home medical provider during their participation. The care team will then help patients and their loved ones understand and plan for the next steps after they are discharged from the program, including planning for upcoming doctors’ visits, changes to medication regimen, and what to look out for to make sure the patient stays healthy. Patients and families can ask their care team if Hospital at Home is an option for them.
