Requesting Access - Overview

EpicCare Link is a web application that allows community-based providers and collaborators to view clinical data, receive Event Monitoring notifications (ex: ED Admission, ED Discharge, Abnormal Lab Results, etc.) on patient, place outpatient Referral Orders, and view patient charts. EpicCare Link is utilized by referring patients to a New York Presbyterian Hospital facility, such as referrals to the ED, Radiology or Lab outpatient orders, etc. New York-Presbyterian, Weill Cornell Medicine and Columbia Doctors grant access to community based providers and business entities by means of having a Consortium Site agreement or Tripartite Business Associate Agreement signed.

Requesting Access

Complete the Smartsheet access request form below for yourself and/or your office staff. Please note we allow only two office staff members’ access per physician. If you are completing the form for office staff please enter the physician information in the Legal Agreement Information for access section within the form.

Sign Legal Site Agreement

The agreement will be electronically sent to physicians email address from email, [email protected] once form is submitted and sponsor has approved. Once the access request form is submitted, the legal agreement is signed and sponsor approval is received the Provider Services Organization team will initiate the CWID (username) creation process for each user and will assist with the setup of Duo mobile for user authentication. 
     

Next Steps

Instructions will be sent to the user's email address with their CWID, how to enroll in DUO and the training link to be completed in Workday.

 

Training Completed

Log into EpicCare Link from your office after training is completed.
