The Westchester County Department of Health and Westchester County Hospitals are conducting a Joint Community Health Needs Assessment.
The purpose of taking the Westchester County Community Health Survey is to get your opinion about Community Health concerns in Westchester County. Results from this survey along with other information will be used to select health priorities for 2016-2018.
