Family Planning Practice

The Family Planning Practice supports pre-conception health and improved birth outcomes to prevent unintended pregnancy, reduce the transmission of STIs, and promote overall well-being through preventive health care and health education.

A Safe Haven

"Norma,” a new 17-year-old patient, came to the Family Planning Practice to talk about getting birth control for the first time. Noticing her apprehension, the front desk staff swiftly referred her to one of our Health Educators, who put her at ease that she was in a safe space and that any conversation would be confidential. Norma was especially grateful for being informed about her rights as a minor to seek confidential sexual and reproductive healthcare. She was referred to Family Planning by her boyfriend, a patient at the Young Men’s Clinic, and she asked for him to be present while she decided on a birth control method. The teen-friendly counseling made Norma and her partner feel confident, safe, and proud of their responsible shared decision-making. Norma then saw a provider who gave her the contraception she had chosen, and the young couple left together after thanking the team.

Project STAY

Project STAY (Services to Assist Youth) provides comprehensive, integrated health services for youth living with, or at-risk for, HIV and STIs.

DB's Story

DB was a 22-year-old cisgender African American gay male who was screened for HIV and STIs by our outreach team at a PRIDE event. His HIV test was negative, but he screened positively for rectal chlamydia. He was referred to a NewYork-Presbyterian sexual health clinic for PrEP initiation and treatment, and he subsequently also screened positive for syphilis. DB received treatment for both infections, PrEP and DoxyPEP education, counseling, and prescriptions. We also gave him the monkeypox vaccine during his visit.

Sexual Health Mobile Medical Unit

Launched in February 2024, the Sexual Health Mobile Medical Unit (MMU) is a full-service, ADA-compliant HIV and Sexual Health mobile medical unit. It provides gender-affirming, LGBTQ+, and trauma-informed care in a safe and inclusive space, with a focus on addressing HIV and sexual health disparities in the diverse communities of Brooklyn, Queens, Harlem, and Washington Heights. The goal is to improve sexual health and HIV outcomes.

The MMU brings accessible and affordable HIV screening, prevention, and treatment, as well as additional sexual health services, directly to community members at risk for sexually transmitted infections (STIs), HIV, or HCV infection, including those lost to care and/or with significant barriers to engaging in care in traditional medical settings. Full-time MMU staff work with local community-based organizations, NewYork-Presbyterian community and population health programs, and New York City care delivery systems to provide additional targeted outreach. The MMU helps ensure that NewYork-Presbyterian supports New York City’s End the HIV Epidemic (ETE) initiative.

The Comprehensive Health Program

The Comprehensive Health Program (CHP) provides integrated equity-focused, status-neutral, gender-affirming care to individuals who are living with HIV, at risk for HIV, or in need of sexual health services or general ambulatory infectious disease services. CHP incorporates primary care with specialty infectious disease services as well as behavioral health and substance use disorder services provided by multidisciplinary teams.

CHP is a New York State-designated AIDS Center, a National Committee for Quality Assurance-designated Patient-Centered Medical Home (PCMH), and the only HIV program in New York State's HealthHome model to function as a HealthHome Case Management Agency. CHP serves communities most impacted by HIV, especially populations at increased risk for poor health outcomes due to stigma, structural racism, criminal justice involvement, serious and persistent mental illness, substance use, psychosocial trauma, or poverty, or are disadvantaged in terms of social determinants of health.

Our teams are fluent in English and Spanish and care for all communities, with immediately available interpreter services. Care is also offered on a 40-bed NewYork-Presbyterian inpatient unit.

HIV and Aging (CHP "50+") aims to improve outcomes among people age 50+ living with HIV, with particular emphasis on wellness and quality of life

  • HIV and the Enhanced Data to Care Model: Investigated 528 patients deemed to be out of care and successfully confirmed linkage or transfer to care for >66% of them. Verified immediate anti-retroviral therapy for 20 patients, 328 STI screenings, 68 patients successfully treated, and 1,750+ successful supportive service referrals and client navigation services.
  • HIV and Aging: CHP's 50+ wellness enhancements included integrating comprehensive geriatric screenings for quality of life, cognitive function, health literacy, and IT literacy/access; supportive care coordination; iPad loaner and tech education program to bridge the technology gap and optimize telemedicine and telepsychiatry services; addition of onsite neurology and physical medicine/rehabilitation specialties, along with nutrition and psychiatry; wellness support groups; and hiring of 50+ client Wellness Champions.
