Addressing Social Determinants of Health (SDoH)

Tackling issues such as housing insecurity, food insecurity, and transportation challenges is a crucial part of NewYork-Presbyterian’s strategy to promote greater equity within our communities. Over the past few years, NewYork-Presbyterian has advanced several initiatives in this area, including the adoption of standardized SDoH screening tools, the development of educational resources for teams, the creation of workflows for screening and referral, and the integration of tools within the electronical medical record (EMR) to facilitate social referrals.

NewYork-Presbyterian is recognized as a leader in this field, with numerous publications in academic journals. In 2024, key recognitions include:

  • Served as faculty for Healthcare Association of New York States’ Advancing Healthcare Excellence and Inclusion learning collaborative, launched with generous support from the Mother Cabrini Health Foundation.
  • Selected to present at Epic’s Users Group Meeting to showcase our SDoH strategy and its integration within the EMR.
  • Highlighted by the Joint Commission’s Health Equity Research Center as an exemplar in SDoH screening and referral.
  • Invited to speak about SDoH at multiple national conferences, including Association of American Medical Colleges (AAMC), Becker's Hospital Review’s Annual Conference, and the Health Care Improvement Foundation, among others.

Measuring Health Equity

At NewYork-Presbyterian, we prioritize equitable care through data-driven health equity measurement. We identify access, quality, experience, and outcome measures aligned with our organizational priorities, disaggregating them by key demographics such as race, ethnicity, language, and payor status, with plans to expand to include sexual orientation/gender identity (SOGI) and health-related social needs.

This approach helps us identify disparities and implement targeted interventions. We are committed to addressing any disparities we uncover and using our data to improve the system as a whole. By being transparent about our results, we maximize opportunities for improvement, help ensure accountability, and track the progress of our equity initiatives. Health Equity Report

The Dalio Center led the drafting, collation, and publication of the 2024 NewYork-Presbyterian Health Equity Report, which assessed the demographic profile of inpatient discharges, emergency department visits, and outpatient visits at NewYork-Presbyterian, as well as highlighted our SDoH strategy and programs.

Data Analytics Training Course and Compendium

In May 2024, the Dalio Center’s data analytics team developed a health equity data curriculum to introduce NewYork-Presbyterian’s broader analytics team to key health equity concepts. The training covered our methods for collecting and standardizing sociodemographic data elements, as well as strategies for data disaggregation and health equity measurement. To support this curriculum, the team created a 15-page guide titled "Health Equity Analytics: Data Elements Guide," which includes descriptions of data elements, data collection methods, and helpful information on locating sociodemographic data in our electronic health record system.
