Update on COVID Vaccines from ColumbiaDoctors, Weill Cornell Medicine, and NewYork-Presbyterian
Dear Valued Patient,
We are currently able to vaccinate those eligible individuals age 5 and over who live in the United States. Vaccination appointments are required.
What You Can Do
- Check to see if you are eligible:
- Are you age 5 or older?
- Do you live in the United States?
- If you answered yes to all of these questions and want to make an appointment:
- Make sure you are registered on the Connect patient portal. If you do not have an account, click here for a guide.
- You will then be able to log into Connect to search for an appointment time.
- Local residents can also call (646) 838-0319 to schedule their appointment for community members age 5 and over. The hotline is open Monday – Friday 9:00 a.m. to 4:45 p.m. to residents of the following zip codes: 10026, 10027, 10029, 10030, 10031, 10032, 10033, 10034, 10035, 10037, 10039, 10040, 10451, 10452, 10453, 10454, 10455, 10456, 10459, 10460, 10461, 10462, 10463, 10464, 10465, 10472, 10473, and 10474
- Make sure that your personal information is up to date. We will use the Connect patient portal to communicate the latest news and alerts through Connect.
