Family Health Center of Western Queens is a multidisciplinary
primary health care center located at 36-11 21st Street,
Long Island City, New York 11106. The comprehensive services available
include health screenings, physical examinations and acute and
chronic health care management.Ê The bilingual (English/Spanish)
full-time social worker provides services such as referral and
liaison, health education and information, mental health assessment,
counseling and support groups. The social worker, Jane Weiler,
CSW is availableÊ Monday through Friday from 9 a.m. to 5 p.m.
