Have you ever experienced or witnessed a traumatic event?

Examples include:

  • Military combat
  • Natural disasters, such as a hurricane, flood, or earthquake
  • Manmade disasters, such as terrorist attacks or industrial accidents
  • Serious accidents, such as a burn injury, fall, car/motorcycle accident, train wreck, or plane crash
  • Physical or sexual abuse
  • Interpersonal violence, such as carjacking, mugging, rape, torture, or kidnapping

Do you have disturbing thoughts, memories, or nightmares of your trauma?

Do you have difficulty remembering important aspects of the traumatic event?

Do you find you are using drugs or alcohol more to help cope with anxiety?

Do you ever feel as if you are reliving your traumatic event through flashbacks or vivid memories?

Is it difficult for you to stay focused?

Do you avoid certain people, places, or things that remind you of your traumatic experience?

Have you lost interest in activities or feel numb?

Do you feel jumpy, on guard, or easily startled?

Do you find yourself feeling more irritable or angry since your traumatic event?
