The NewYork-Presbyterian Community Health Plan is a managed care company that was formed in 1995. Our original name was The New York Hospital Community Health Plan. The name was changed in 2002 to reflect the merger of The New York Hospital and Columbia Presbyterian Medical Center.

As a member of our plan, you and your family will have access to a network of some of the leading physicians and hospitals in your community. You probably already know us, we are affiliated with the NewYork-Presbyterian Healthcare System (the University Hospitals of Columbia and Cornell), the Boriken Neighborhood Health Center and many other highly qualified hospitals, health centers and physicians throughout Brooklyn, Queens, Manhattan and the Bronx. All of our affiliates are committed to delivering high quality healthcare services to you and your family 24 hours a day, 365 days a year.

We offer Child Health Plus, Family Health Plus and Medicaid Managed Care. Enrollment is easy and we have people ready to assist in the enrollment process.
