New Patient Forms

If this is your first visit to our clinic, you will need to fill out a New Patient Form. For your convenience, these are available online (see below), or you may request a form from our main office. Please bring the completed forms with you to your appointment. This information will provide your healthcare team with valuable information to better treat your child. If you have any questions, please feel free to contact us.

Location:

We are located at the NewYork-Presbyterian Phyllis and David Komansky Center for Children's Health in the Helmsley Medical Tower building located at the corner of York Avenue and East 70th Street. Depending on your appointment, you will be directed to either the third or fifth floor.

Our office is located on the 3rd Floor within the Pediatric Sub-Specialties Department in the Helmsley Medical Tower at 505 East 70th Street (between York Avenue and the East River) in Manhattan.

Check in:

Once at our office, please check in at the front desk. You will be asked to complete some basic registration forms. Please make sure to have your insurance information with you. If a referral is needed, please bring a copy with you. Co-pays or full visit payment will be collected at the time of check out.

After registration:

One of our nurses will call you or your child to obtain a weight and height. Sometimes they will measure the head size and sometimes a blood pressure will be taken. After the nurse obtains these basic measurements, she will bring you back to an exam room.

Medical examination:

One of our medical staff members will talk with you to obtain a full medical history. Please remember to bring any appropriate medical records and any imaging studies that will be pertinent for the visit. The history obtained will be reviewed with Dr. Kaleb Yohay. An in depth-neurological exam will be completed as well. Following a full physical exam, we will sit and talk further with you about any questions or concerns you may have. We will also discuss with you any immediate management issues and a plan for future care and follow up.
