As a cooperative effort between The NewYork-Presbyterian Community Health Plan and Child Health Plus (a New York State subsidized health insurance program for uninsured or underinsured children), the program's goal is to provide comprehensive health care for children of families that do not receive health insurance benefits from their employers and who cannot afford private insurance coverage.
The NewYork-Presbyterian Community Health Plan is a quality driven Health Maintenance Organization (HMO). As plan members, your children will have access to a network of the leading doctors and hospitals in your community and in the country.
If your child is under the age of 19, not eligible for Medicaid, and has limited or no health insurance, this program may be for you.
Who May Join?
A child is eligible for The NewYork-Presbyterian Community Health Plan's Child Health Plus program if he or she:
Is under age 19 |

Is a resident of
Brooklyn, Queens, Manhattan or the Bronx (you and your child do not
have to show proof of U.S. citizenship, but must be a resident of
New York State to apply) |
|
Is not eligible for Medicaid |
|
Does not have equivalent health insurance coverage |
