HealthOutreach ®

HealthOutreach® is a free membership program for people 60 years of age and older that provides a centralized resource to access hospital and community programs. Through membership in HealthOutreach®, seniors have access to:

  • Monthly health information programs, with focus on healthy aging for seniors
    Health Outreach Newsletter
  • Cultural events and classes to enhance wellbeing and decrease isolation
  • Assessment of needs by the HealthOutreach® social work staff; referral to NewYork-Presbyterian departments and services throughout the community
  • Ongoing support to family and friend caregivers through the Caregivers Service
  • Insurance policy information and assistance with claim problems

To become a member of HealthOutreach®, you just need to complete a simple application form. Once we receive your application, you will begin to receive program announcements of all upcoming programs and services. The announcements go out every two or three months, in time for members to call and reserve a space at the lectures, classes, and other HealthOutreach® activities.

Lifeline Personal Emergency Response Service (PERS)

HealthOutreach® administers a fee-for-service Personal Emergency Response Service (PERS), called Lifeline. NYP Social Work staff install the equipment in the home and train you to use it. When there is an emergency, all you need to do is press the waterproof button worn around your neck or wrist. The Emergency Response Center is alerted within seconds, and responds through your Lifeline equipment, to determine what help is needed.

Lifeline is the first to offer the "Auto Alert", which does not require you to press a button. It automatically detects a fall and sends the signal to the Emergency Response Center. This is a crucial service for anyone who: lives alone, has balance issues or a history of falls, lives with chronic illness, such as Diabetes, Congestive Heart Failure, COPD; or has a child with chronic illness.

The charge is $75 for installation and $40/month for service for basic service, $53 a month for Auto Alert. Lifeline is available in Manhattan, Queens and parts of Brooklyn and the Bronx.

The Caregivers Service

The Caregivers Service is a free program of HealthOutreach®, dedicated to meeting the needs, challenges, and concerns of family members and friends who care for older adults (60 years old and above). The Caregivers Service offers educational seminars, lectures, information and referral, as well as a newsletter. The Caregivers Service also offers two support groups: one specifically for caregivers of people with Alzheimer's and other cognitive impairments, and the other is or Caregivers of anyone 60 years of age and older.

We are proud to offer The Caregivers Toolkit, which includes important information about Resources, Self Care, Advance Directives and other things caregivers need to know.
