Center for Community Health Navigation (CCHN)

The Center for Community Health Navigation encompasses the Community Health Worker (CHW) and Patient Navigator initiatives across five campuses of NewYork-Presbyterian Hospital and the surrounding communities.

WIN for Diabetes

WIN for Diabetes CHWs provide culturally sensitive support and education to adults with type 2 diabetes and their caregivers. This program is associated with improved health outcomes. go to page

Community Health Workers (CHWs) Integrated in the Patient Centered Medical Home (PCMH)

CHWs are valued members of the health care team within multiple Patient Centered Medical Homes where they provide education and support and bring the community perspective into the clinical setting.

ED Patient Navigator Program

Patient Navigators are members of health care teams in 5 emergency departments of NewYork-Presbyterian Hospital where they provide culturally sensitive support and connect patients to care. go to page
