Mission

The Center for Community Health Navigation (CCHN) at NewYork-Presbyterian Hospital is dedicated to support the health and well-being of patients through the delivery of culturally-competent, peer-based support in the emergency department, inpatient, outpatient, and community settings with the ultimate goals to promote healthcare self-management and to decrease avoidable system utilization.

Scope

CCHN will achieve this mission through four key activities:

  1. Connect Patients to the Health Care System and to Local Community Resources
  2. Support Innovative Patient-Centered Care Initiatives
  3. Advance the Community Health Worker Role and Workforce
  4. Enhance the Knowledge Base and Inform Local Practice
