Congestive Heart Failure Volunteer Intervention Program (CHF-VIP)

NewYork-Presbyterian Brooklyn Methodist's Congestive Heart Failure Volunteer Intervention Program (CHF-VIP)* enhances patients' understanding of congestive heart failure and reinforces practical techniques to help manage their disease. CHF-VIP Volunteers partner with patients and NYP Brooklyn Methodist professional staff members to bridge the gap in patient information through education. As a CHF-VIP volunteer, you will use health literacy techniques to interview CHF patients to assess the patient's understanding of the disease. You will discuss the importance of medication regimens, weight management, diet adherence, and consistent follow-up with physicians. You will support the patient, both in the Hospital and at home, with follow-up phone calls. Your work is designed to boost patient involvement and awareness, with the goal of reducing rates of re-hospitalization. All volunteers receive training from a vice president for nursing, the director of non-invasive cardiac imaging, and a social worker.

*A research study has demonstrated that the work of CHF-VIP volunteers can help lower readmission rates.

We would love to hear from you if you are: interested in cardiology, public health and education, friendly, supportive and mature, have previous patient care experience, have good communication skills, and the ability to interact with a range of people

Minimum Commitment: Eight to ten hours a week for six months, with at least four of those hours falling between 8:00 a.m. and 4:00 p.m., Monday through Friday.

Times: Seven days a week between 8:00 a.m. and 4:00 p.m.
